Monash Uni Soccer

MUSC members are covered by two insurance policies. One provided by JLT through Football Federation Victoria (FFV) and the other provided by Monash University. Each insurance policy has its own set of conditions and depending on the type of claim and amount of money spent/being sought the coverage is different. Please read the information on the FFV and Monash University websites carefully to ensure you are following the correct claim process.

Monash University process summary

1. Read the Monash University personal accident insurance policy to see if you are covered.
2. If you believe you could be covered, follow the instructions on the above web site to report the injury (generally within 24 hours).
3. Once you have paid all of your costs, lodge the claim with Monash University or as directed.

FFV process summary

1. Read the FFV insurance policy to see if you are covered.
2. If you believe you could be covered, complete an FFV incident report.
3. Once you havepaid all of your costs, complete an FFV claim form.
4. Complete the form as directed and return it to FFV.
